The projects aims at multiple users with different needs and concerns (nurses vs. doctors vs. patients vs. families, etc.). The project aims to determine what role patient documentation takes and how it differs depending on stakeholder involvement. It designs alternative ways to provide access to information and documentation for every person in need of recent data.
Mobile care and support workers face heavy workloads and a lack of information sharing with other health professionals. Information and communication technology support can improve home care and all stakeholders, but its full potential has yet to be realized. The Linked Care project aims to develop information technology systems that address practice-based challenges, increase digital data availability, and reduce work-related burdens on health professionals and clients. Experience with existing IT systems, including the Austrian Electronic Health Record (ELGA), will serve as a starting point to enable end-to-end information provision in mobile healthcare as part of the project. The solution will enable interdisciplinary support of relevant stakeholders in the care process and provide a novel, responsive user interface. Linked Care will be developed based on the needs of the end users and specify the processes in workshops with local representatives and regional administrators.
The solution, therefore, enables the exchange and evaluation of health data via standardized interfaces throughout Austria, as a software portal is created that is connected to existing digital systems. In summary, Linked Care is the digital solution that needs to be implemented in mobile care and support, as it enables precise and fast transfer of information and communication.
